Анотація

Протягом останніх десятиліть опубліковано велику кількість досліджень, які підтвердили роль кишкової мікробіоти в патогенезі функціональних захворювань кишечника. Розглянуто основні механізми, за допомогою яких мікроорганізми, котрі колонізують травний тракт, сприятливо впливають на організм людини. Наведено результати експериментальних досліджень, які підтвердили здатність різних бактерій провокувати розвиток синдрому надлишкового бактеріального росту, синдрому подразненого кишечника і функціонального запору. Розглянуто доказові дані, котрі підтверджують роль кишкової мікробіоти у прогресуванні целіакії. Проаналізовано ефективність різних способів відновлення еубіозу кишечника: за допомогою застосування антибіотиків, які не адсорбуються (рифаксиміну), пробіотиків і трансплантації фекальної мікробіоти. Представлено дані щодо доцільності застосування рифаксиміну при синдромі подразненого кишечника. Наведено доказові дані, які підтверджують наявність у рифаксиміну, крім бактерицидних і бактеріостатичних властивостей, притаманних багатьом антибіотикам, еубіотичної активності. Розглянуто результати метааналізів, в яких вивчали ефективність різних пробіотиків у лікуванні функціонального запору. Наголошено на неоднозначності доказової бази щодо доцільності проведення трансплантації фекальної мікробіоти з метою нівелювання рефрактерного синдрому подразненого кишечника. Доведено доцільність включення еубіотиків у схеми лікування функціональних захворювань кишечника. Необхідно провести дослідження для визначення перспектив лікування дисбіозу кишечника при функціональних захворюваннях травного тракту за допомогою пробіотиків.
