Запальні захворювання кишечника (ЗЗК) є однією з найскладніших проблем у сучасній світовій внутрішній медицині в цілому і в гастроентерології зокрема. Захворювання демонструють рецидиви та прогресування, суттєво знижують якість життя, фізичну активність хворих, спричинюють ускладнення з боку інших органів і систем людського організму, зниження або втрату працездатності, труднощі в соціалізації та створенні сім’ї. Недостатньо прогнозований перебіг захворювання, гострі чи хронічні кишкові ускладнення можуть призвести до оперативних втручань. За тяжкістю перебігу, частотою інвалідизації, ускладнень, зокрема таких, що потребують хірургічного втручання, за економічними витратами системи охорони здоров’я ЗЗК випереджають інші поширені захворювання органів травлення. Різноманітність клінічних, анатомічних (зокрема локалізація та протяжність ураження травного каналу) та ендоскопічних ознак хвороби в поєднанні з різною тяжкістю й активністю захворювання, наявністю локальних і системних позакишкових виявів зумовлюють труднощі зі швидким встановленням діагнозу, диференційною діагностикою та вибором ефективної і безпечної терапії, яку призначають не лише для досягнення стійкої ремісії захворювання, а й для її збереження.
У більшості пацієнтів захворювання характеризується хронічним перебігом із рецидивами та потребує тривалого, постійного, часто пожиттєвого медикаментозного лікування із застосуванням препаратів різних фармакологічних груп, зокрема комбінованої терапії. Триває активний пошук прогнозовано ефективних лікарських засобів для лікування ЗЗК. Згідно з новою міжнародною класифікацією хвороб 11‑го перегляду (2022), до групи ЗЗК віднесено три захворювання: виразковий коліт (ВК), хворобу Крона (ХК) і невизначений коліт, тобто захворювання, за якого неможливо чітко визначити, чи це ВК, чи це ХК ободової кишки. У сучасній медицині спостерігається тенденція до більшої чіткості та лаконічності при формулюванні діагнозу, в алгоритмах обстеження та схемах лікування. Запальні захворювання кишечника залишаються захворюваннями з багатьма невідомими як щодо встановлення чіткого діагнозу, так і щодо прогнозу перебігу та передбачуваності наслідків медикаментозної терапії захворювання.
