Анотація

Такі поширені функціональні шлунково‑кишкові розлади, як синдром подразненого кишечника (СПК) і функціональна диспепсія (ФД), хоча і не представлені будь‑якими очевидними структурними ураженнями шлунково‑кишкового тракту, але значною мірою впливають на якість життя численних пацієнтів. За епідеміологічними даними, 26,7 — 48,7 % хворих на СПК і 20,0 — 42,1 % пацієнтів із ФД мають перехрест симптомів (оверлеп‑синдром СПК/ФД). Синдром перехресту зазвичай призводить до серйозніших клінічних виявів, погіршення якості життя і виникнення ускладнень. У патогенезі як СПК, так і ФД велику роль відіграють хронічне низькоінтенсивне постінфекційне запалення або неінфекційне мікрозапалення та імунна відповідь, що призводять до вісцеральної гіперчутливості, порушення функції oсі «мозок —кишечник» та слизового кишкового бар’єра, що зазвичай є причиною симптоматики СПК або ФД. Однак не зрозуміло, чи має запалення такий самий механізм при синдромі перехресту СПК/ФД. Висвітлено останні досягнення у вивченні механізмів запалення при оверлеп‑синдромі СПК/ФД і запропоновано нову тактику лікування. Представлено сучасні дані щодо ролі перенесеної гастроінтестинальної інфекції, вторинного хронічного запалення, імунної відповіді, пошкодження кишкового слизового бар’єра, асоційованого з інфекцією шлунково‑кишкового тракту, впливу шлунково‑кишкових інфекцій на ентеричну нервову систему та вісь «мозок — кишечник», можливої ролі інфекції Helicobacter pylori при перехресті симптомів СПК/ФД. Також розглянуто роль неінфекційного запалення, зокрема синдрому надмірного бактеріального росту, харчової алергії, психологічного або ментального стресу у патогенезі СПК, ФД та оверлеп‑синдрому СПК/ФД. Висвітлено сучасну тактику лікування та ведення таких хворих, в основі якої лежить вплив на запальні механізми.
