Постінфекційний синдром подразненого кишечника: останні рекомендації робочої групи Римського фонду

Анотація

Існування такої форми синдрому подразненого кишечника (СПК) як постінфекційний СПК (ПІ-СПК) давно доведено клініко-лабораторними та епідеміологічним дослідженнями, проведеними в різних географічних і клінічних умовах. Проте офіційних клінічних рекомендацій з діагностики та лікування ПІ-СПК тривалий час не було. Для того щоб узагальнити наявні докази з патофізіології, діагностики і лікування ПІ-СПК, Римський фонд створив робочу групу з цього питання, яка представила звіт і рекомендації, котрі ґрунтуються на сучасних даних літератури та клінічному досвіді. Робоча група провела ретельний аналіз баз даних публікацій, які описують клінічні особливості (діагностику), патофізіологію (кишкові сенсомоторні функції, кишкова мікробіота, імунна дисрегуляція, бар’єрна дисфункція, ентероендокринні шляхи, генетика) і тваринні моделі ПI-СПК. При цьому використовували систему консенсусу на основі Delphi для створення клінічних рекомендацій з ведення хворих із ПІ-СПК, котрі ґрунтуються на даних доказової медицини та клінічному досвіді членів цієї групи. ПI-СПК розвивається приблизно у 10 % пацієнтів після перенесеного інфекційного ентериту. До чинників ризику належать жіноча стать, молодший вік, психологічний дистрес під час або перед гострим гастроентеритом, тяжкість гострого епізоду. В патогенезі ПI-СПК мають значення зміни в мікробіомі кишечника, а також чинники епітеліальної, серотонінергічної та імунної системи, хоча самі механізми розвитку та прогресування ПІ-СПК не повністю зрозумілі. Оскільки специфічні лікувальні рекомендації для ПІ-СПК не розроблено, його лікування аналогічне терапії СПК. Наведено алгоритм діагностики та лікування ПI-СПК. Запропоновано напрями майбутніх досліджень.
