Сучасні підходи до моніторингу та лікування виразкового коліту в дітей: огляд оновлених практичних рекомендацій

Незважаючи на суттєвий прогрес медицини, проблема запальних захворювань кишечника в дитячій популяції зберігає актуальність. Сучасні дослідження сфокусовані на вивченні ролі генетичних чинників, мікробіому кишечника, імунних механізмів запалення в розвитку патології кишечника та раннього впровадження персоналізованих терапевтичних стратегій. До найдискутабельніших аспектів ведення дітей із виразковим колітом належать вибір терапевтичних стратегій (рання ескалація лікування або поетапний підхід) і визначення адекватних терапевтичних цілей ремісії.
Проведено аналітичний огляд сучасних підходів до менеджменту дітей із виразковим колітом на основі оновлених консенсусних рекомендацій, що ґрунтуються на доказах, Європейського товариства дитячої гастроентерології, гепатології та харчування та Європейської організації з хвороби Крона й коліту (2025). В Україні впродовж останніх років, попри складні умови, спостерігається стійка позитивна динаміка в організації медичної допомоги пацієнтам із запальними захворюваннями кишечника. Удосконалюються клінічні протоколи, підвищується рівень професійної підготовки медичних працівників, поступово розширюється доступ до сучасних діагностичних технологій.
Порівняно з попередніми рекомендаціями підходи до лікування виразкового коліту в дітей істотно оновлено завдяки появі нових доказових даних і вдосконаленню терапевтичних стратегій, зокрема ширше впроваджують ультразвукове дослідження кишечника, зростає роль терапевтичного моніторингу лікарських засобів при застосуванні інгібіторів фактора некрозу пухлин, формується практика використання вищих доз препаратів і комбінованих терапевтичних стратегій.
Цей огляд є не лише узагальненням сучасних підходів до ведення амбулаторних пацієнтів із виразковим колітом. Створено освітній ресурс — документ, що визначає стандарти лікування з урахуванням відмінностей у доступності медичної допомоги та ресурсного забезпечення.
Огляд оновлених рекомендацій дає змогу практикуючим лікарям ознайомитися з принципами якісної практики європейського рівня та підвищення ефективності спостереження дітей із виразковим колітом з урахуванням регіональних можливостей.
